The Self Assessment eForm is a helpful checklist to assist the research teams and the PAM auditors with examining and monitoring the IRB approved protocol. The eForm is accessible to everyone and can be utilized at any time. Perhaps the most effective way to prepare for a PAM audit is to carefully read the protocol and ensure the research staff are performing the research activities as described and approved by the IRB. Any changes to the protocol must be approved by the IRB prior to implementation by submitting a Modification Form. One small change may seem trivial to a researcher, but it may be of great concern to the IRB, federal regulators, or auditors.
