Application Submission
What to Submit
The investigator or instructor should complete the appropriate Institutional Biosafety Committee (IBC) protocol application found on the Forms and Permits page of this website. The application should be submitted to the Office of University Research Compliance, which is located at 223 Scott Hall. The form can be completed electronically, but a paper copy, complete with signatures and all supporting documentation, is required.
The application must include a project summary, project goals and procedures. The following supporting documents should also be included:
safety and security procedures
description of lab-specific training
an overall risk assessment that includes:
identification of all potential hazards, to include such things as excretion by animals, aerosol generation, use of sharps, etc.
discussion of engineering controls, safety procedures and personal protective equipment that will be employed to minimize risk for exposure.
discussion of accidental spill or exposure procedures specific to your project.
Material Safety Data Sheets (MSDS)
Note: Remember that federal regulations require that the IBC be composed of persons with diverse backgrounds. Not all committee members are scientists. Thus, some members may not be familiar with a specific discipline. Therefore, it is the responsibility of the investigator or instructor to write the project summary in such a way that its technical merit and methodology can be reviewed by persons with diverse backgrounds.
The application must contain all applicable signatures.
All project personnel who are listed on the application must initial by their name acknowledging their responsibilities for the project.
A copy of the most recent lab inspection report must be attached to the application at the time it is submitted to the IBC.
