Frequently Asked Questions
Below you will find our frequently asked questions providing essential information regarding the Conflict of Interest disclosure completion using the COI-Smart system.

How do I access and use COI-Smart?
From the email notification you received from noreply@coismart.com regarding the completing of the disclosure, click on the link to the questionnaire in COI-Smart, or go to https://my-okstate.coi-smart.com/. You will use your Oklahoma State University issued email address and the password you created when you first registered with COI-Smart.
Click "My Questionnaires" and click the 'OSU Conflict of Interest Disclosure Questionnaire".
Proceed through the questions clicking [Save and Continue]. -
I am unable to log into COI-Smart, what should I do?
The system locks you out after 5 failed login attempts in 3 minutes. Please try again after 30 minutes.

How will I know when my disclosures will expire?

Can I save my progress and complete it later, or must I complete the questionnaire
in one sitting?
You can save your progress on the questionnaire and log back in later to finish the questionnaire. Your questionnaire Status will show ''In Progress'' if you have started but not completed the questionnaire. When you log back in, under ''My Questionnaires'' select/highlight the questionnaire you would like to continue and then down below click ''Complete This Questionnaire.''
Below the “Complete This Questionnaire” link you can see all the questions, the status (''Complete'' or ''Pending'') of each and can edit or finish your answers by clicking ''Modify'' or ''Answer.''

How do I go back to a previous question to review or edit my answer?
If you need to go back and review or edit your answer while completing the questionnaire, click on the numbers in the top-left corner of the questionnaire in order to go to the question(s) you would like to revisit. Question numbers that are green have been answered and questions that are gray have not been answered.

How will I know I have submitted the questionnaire and my answers have been received
for review?
Once you have successfully submitted your questionnaire by answering all required questions, typing “submit” and clicking “finish”, you will immediately receive a message that the questionnaire was submitted successfully.
You will also receive an email confirmation from noreply@coi-smart.com stating that your disclosure has been successfully submitted. The Status of the questionnaire will also show ''Submitted'' in COI-SMART.

I have already submitted my questionnaire, but I need to change my answer. What do
I do?

Where can I find definitions of some of the terms used?
Within the questionnaire you may hoover over any of the italicized blue words in order to be presented with a definition of the word.

How can I get a copy of my responses after I submit?
Once you have submitted your questionnaire you will be able to download an Adobe PDF copy of your completed questionnaire.
Additionally, you may log back in, under ''My Questionnaires'' select/highlight the questionnaire you would like to obtain a copy of your responses.
